The safety and security of our New Members is of paramount importance to Delta Sigma Phi Fraternity.
We feel that before we can properly onboard New Members, we must first gather necessary personal information to communicate effectively, discuss potential health challenges (if any), and collect contact information for Parents/Guardians in case of emergency.
PLEASE COMPLETE THIS GOOGLE FORM WITHIN 48 HOURS OF THE FIRST NEW MEMBER MEETING.
Delta Sigma Phi understands the delicate nature of some of the personal information being requested.
  NOTE: Any personal health information will ONLY be shared with the New Member Education Staff, E-Board and Alumni Corporation Board Leadership Team Members (and ONLY IF NECESSARY).  Personal health information will NOT be shared with other Alumni, Undergraduate, InterFraternity Council members.
